Here’s how to receive your mail-in rebate:
Purchase the specially marked Physicians Formula Youthful Wear Youth-Boosting Foundation product. Product must be purchased between 1/1/12 and 12/31/12. Send in completed rebate form along with the TRY ME FREE label attached to the product and your original sales receipt with purchase price circled. Rebate must be postmarked by 1/15/13.
Limit one rebate per household. Rebate amount is the purchase price of Youthful Wear Youth-Boosting Foundation not including tax. Maximum value is $14.95.

There are a few different Physicians Formula products with “Try me Free” Rebates. I know the rebates state “one per household”, but is that for one of each item or one rebate of any Physicians Formula product per household??
