There seems to be 3 types of the enamel health toothpaste, multi-protection (not included in the catalina deal), sensitivity relief, and whitening.

So, the sensitivity relief and whitening types should trigger a Catalina print? What should I do if I didn’t get one? 🙁
That’s correct, if you didn’t get one, you can call 888-826-8766 within 14 days of your transaction and tell them, they will either tell you why you didn’t get one, or send you it in the mail. Sometimes catalinas are regional which means only certain areas will get the deal.
Out here in WA we’ve been doing this deal like crazy during the past two days where the toothpaste is $2.99 on sale and so is free with Sunday’s coupon. One thing to note in your post above:
THE MULTI-PROTECTION ENAMEL HEALTH TOOTHPASTE IS EXCLUDED FROM THIS PROMO. You will NOT get the $2 catalina if you purchase it.
Also, the Optic White toothpaste that is $3.49 at Kroger ($2.99 here) does not match Sunday’s SS coupon so it cannot be applied to that toothpaste.
