- Product codes from your two (2) qualifying Marcal® Small Steps® product purchases.
- Cash register receipt from your one (1) Phineas and Ferb The Movie: Across the 2nd Dimension 2-Disc DVD purchase imprinted with store name and dates between August 28, 2011 and December 1, 2011 with purchase price circled (photocopies acceptable).
- Cash register receipt from your two (2) qualifying Marcal® Small Steps® product purchases.
- Hand printed, completed rebate certificate or a 3 x 5 card with your name and complete address (no P.O. Boxes) including telephone number.
- Requests must be postmarked by January 1, 2012.
